Open-Wedge and Closed-Wedge High Tibial Osteotomy: Current Concept and Long-Term Results

Abstract
A high tibial osteotomy (HTO) is a joint-preserving surgery for symptomatic medial compartmental cartilage or meniscal disorders in the presence of lower extremity varus malalignment. The benefits of HTO over joint replacement procedures include joint preservation and the potential to help patients maintain higher activity levels. Both closed-wedge (CW) and open-wedge (OW) HTO showed excellent postoperative satisfaction. CWHTO achieved better bone union. OWHTO has the advantage of the ease of alignment correction and reduced risk of peroneal nerve palsy. Based on past literature, the 10-year survivorship of CWHTO and OWHTO ranged from 53.0%?89.3% and 81.7%?90.0%, respectively. Therefore, the appropriate surgical method should be selected according to the condition of the patient's knee.
